What is Low Testosterone?

 What is Low Testosterone? 

A common question we get from patients who wish to learn more about their health and how Low T can effect their overall health and happiness.

Testosterone is primarily a male sex hormone made in the testes, however females also produce testosterone, just in smaller amounts. Testosterone is vital to the normal sexual development and functioning in men and remains important through a males life. 
When males reach puberty, increased testosterone development helps to develop masculine features such as body and facial hair, deepening of the voice, and muscle  growth with increases in strength. Males also need testosterone to produce sperm. Testosterone levels in men will begin decrease with age around the mid 20's and steadily decline around 1- 2% annually. Some men will begin to notice the low testosterone symptoms in their 30's or 40's. 
For men that have low testosterone levels the first step is to get tested and diagnosed for Low T. Low Testosterone is often called Low T, or TD (Testosterone Deficiency) Sometimes it may be referred to as hypogonadism or andropause. The American Urology Association has identified Low T as having less than 300 nanograms per deciliter (ng/dL) serum (total testosterone) on 2 separate tests obtained during the morning, when levels are at their highest. 

Low Testosterone Symptoms 


A list of some of the more common symptoms of low testosterone includes;


Feeling tired, lack of energy and fatigue
Decreased libido and sex drive 
loss of lean muscle mass
Mod swings and irritability
Feelings of anxiety or depression
Lack of self confidence
Poor sleep quality
ED


There are many other possible reasons for these symptoms of Low Testosterone besides age. Alcohol and narcotics use, some congenital medical disorders, testicular cancer, and chronic medical conditions like diabetes and obesity can disrupt the endocrine system. Stress and lack of sleep may also contribute heavily to decreased testosterone levels.

HCG (Human Chorionic Gonadotropin) is often called the pregnancy hormone due to the fact that it’s made by cells formed in the placenta, which provides nutrients to the egg after it’s been fertilized and attaches to the wall of the uterus. HcG levels may first be detected in a blood test around 11 days after conception and approximately 12-14 days after conception if taking a urine test. HCG is an FDA-approved medication that may help treat specific conditions for both men and women. For men, it plays an important role in TRT.

 


What is HcG used for in men?

For many male patients, hcg may be prescribed to help treat the symptoms of hypogonadism often called low T or Male Andropause. It can also be used to treat infertility. HcG works to increase the males natural production of testosterone in the testes and increases the production of sperm.

Human chorionic gonadotropin is a hormone that’s composed of 2 subunits. An α and a β-subunit. hCG and Luteinizing hormone, or LH, both share an identical α-subunit. hCG  if different from LH because of the fact that its β-subunit with a 24 amino acid tail. This 24 amino acid side chain helps to increases the receptor activity and will slow the metabolism of hCG in the body. HcG has a half life that is approximately 36 hours compared to 30 minutes for Luteinizing Hormone. Because they both share the same receptor, have increased receptor activity, and have a much longer half, life, hcg is the best pharmacological analog to LH.

HCG injections are frequently prescribed to men with testosterone deficiency. Normal ranges of testosterone in men are often 250 to 1100 Ng/dl or nanograms per deciliter. With suboptimal testosterone levels and the physical/emotional symptoms of low testosterone, a physician may be able to diagnose a patient and treat them for hypogonadism.

These symptoms may include:

  • Constantly feeling tired and fatigued
  • Internalizing and feeling greater levels of stress
  • Low “Drive” and lack of romantic interest
  • Depression
  • Anxiety
  • Lack of confidence
  • Increased Body fat
  • Decreased muscle mass
  • Poor sleep quality





The American Urological Association recommends, hCG for patients who have been diagnosed with testosterone deficiency and that also desire to maintain fertility and natural testosterone production.

HcG injections can help increase testosterone levels in men, as well as increase fertility, “romantic drive” and increase the size of the testes, which is important for patients on TRT who may experience atrophy, or “shrinkage” as its called. It helps increase the effectiveness of testosterone therapy by helping maintain stable levels, natural production and prevents some side effects associated with testosterone injections.

How does it work to increase testosterone?

In men, hCG acts as a luteinizing hormone (LH) analog. LH stimulates the Leydig cells which are located in the testicles. This then results in the production of testosterone within the testes, keeping natural production working and preventing testicular shrinkage. LH also will stimulate the production of sperm within the seminiferous tubules of the testes and as both testosterone and sperm are being produced, the testicles will maintain their size for patients on TRT.
