Through the gradual increase dosing of semaglutide, side effects can be mitigated. 

The safety and effectiveness of semaglutide has been studied in for separate trials, which were randomized, double blind, placebo controlled  , randomized, withdraw trial, in which patients receiving Semaglutide would either continue with treatment or switched to a placebo. 

In the trials, the average age of the patient starting Semaglutide was 46 years and 74% of the patients were female. The average body weight was 231 pounds with an average BMI of 38. Patients who receive Semaglutide lost an average of 12.4% of their initial body weight.

To properly mix and inject HCG for Testosterone therapy, follow these steps:

 

1. Begin by washing your hands thoroughly with soap and water for at least 20 seconds to ensure cleanliness.

 

2. Gather all the materials you need for the process. This includes bacteriostatic water, HCG vials, a reconstitution syringe and needle, alcohol wipes, and a Sharps container for safe disposal. If you plan to inject after reconstitution, prepare a second syringe, needle, and alcohol wipe to avoid contamination.

 

3. Prepare all of the materials by removing the caps from the bacteriostatic water and HCG vial. Sterilize the tops of both vials using an alcohol wipe. These can be found at just about any local pharmacy such as a CVS or Walgreens. Attach the needle to the syringe, being careful not to touch the needle with your fingers. Avoid letting the needle rest on any surface or come into contact with anything to prevent contamination.

 

4. Draw up the appropriate amount of bacteriostatic water into the syringe. The amount of water needed depends on the size of the HCG vial and the desired concentration. For example, a typical 10,000iu vial, you'll need 10 mL of bacteriostatic water. Pull air into the syringe up to the required amount, then pierce the stopper of the bacteriostatic water vial. At this concentration a 500iu injection would be 0.5mLs of hCG, which measures 50 units on the provided insulin syringe. You can also mix a 10,000iu vial with 5 mL of bacteriostatic water. At this concentration, the same 500iu injection would only be a 0.25 mL injection. By adding half the water, you’ll increase the concentration of medication and require a smaller volume of medication per injection but still receive the same amount of hCG per injection.

5. Slowly inject the air into the vial, allowing the syringe to adjust if you feel any pressure. Press the plunger again until all the air is out of the vial and the bacteriostatic water is in the syringe at the appropriate amount. Avoid any contact between the needle and the HCG vial to prevent contamination.

 

6. Pierce the stopper of the HCG vial with the needle of the syringe containing the bacteriostatic water. Slowly push the water into the vial, taking caution and stopping if you feel pressure. Often these vials are vacuum sealed and will draw the water in automatically due to the difference in pressure. It will make it easier to draw the hCG out of the vial and into the thin insulin needle for injection later if the vial of hCG is slightly pressurized. This can be done by injecting a little extra air into the bottle until you feel some light resistance on the plunger. Be careful to not over pressurize the vial as this can cause you to have difficulty measuring the medication when filling the syringe.

 

7. Gently swirl the solution in the HCG vial until the powder and bacteriostatic water are fully combined. Avoid shaking or aggressively mixing the medication. Once the powder is fully dissolved, there should be no visible powder in the vial.

 

8. If you are ready to inject the HCG, use a new needle, syringe, and alcohol wipe. Follow the guidance of your medical provider and pharmacy for administering subcutaneous injections, as HCG injections are typically done subcutaneously.

 

9. After reconstitution, immediately refrigerate the HCG. Prior to mixing, the powdered HCG can be kept at room temperature, but once the bacteriostatic water is added, it should be stored in the fridge. Reconstituted HCG can generally be used for up to 60-90 days from the date of reconstitution.

 

10. In the event that you accidentally leave the HCG out of the refrigerator, it is likely still usable, but avoid leaving it unrefrigerated for more than 72 hours and never in heat or direct sunlight.

Recently ,on March 23rd, 2020 Human chorionic gonadotropin, or HCG,  was designated as one of several medications that the FDA will now consider “Biologic Products”. This means that local compounding pharmacies who fall under the 503A pharmacies under the FD&C Act are no longer able to compound/make HCG without a special “Biologics License”. We are seeing TRT clinics switching to Clomidalso known as clomiphene citrate which is often prescribed for infertility. Also Gonadorelin is being substituted for HcG and is a man-made protein which causes the pituitary gland to release other hormones (luteinizing hormone [LH] and follicle-stimulating hormone [FSH]), which stimulates testosterone production in men on TRT. However research shows that it due to the short half life of this medication which can be between 2 to 10 minutes (vs 36 hours for HcG), injections a minimum of 6 times daily are needed have a similar clinical effect as two 500iu injections of HcG
