Do You Need to Be on a GLP-1 Agonist Medication Like Wegovy, Ozempic or Mounjaro for Life?
Recently there has been an increased interest in the use of weight
loss medications across the US. As a result, the rise of GLP-1 agonist
medications like semaglutide
or tirzepatide,
better known by their brand names Wegovy, Ozempic,
and Mounjaro,
has sparked considerable interest and debate among healthcare professionals.
Initially these medications were developed for diabetics for the management of
type 2 diabetes, however these medications have recently gained traction as
powerful tools in weight loss and obesity management. Wegovy, for instance, is a brand of
semaglutide specifically approved for chronic weight management in individuals
with obesity or those with weight-related conditions. It was approved by the U.S.
Food and Drug Administration (FDA) for chronic weight management on June 4,
2021. It was approved for use in adults with obesity (BMI ≥30) or overweight
(BMI ≥27) with at least one weight-related condition, such as high blood
pressure, type 2 diabetes, or high cholesterol. But with the effectiveness of
these drugs in helping people shed significant weight comes an important
question: once you start taking a GLP-1 agonist, do you need to be on it for
life?
This article will explore the role of GLP-1 agonists for
patients in weight management, what happens to you when you stop taking them,
and whether or not they need to be a lifelong commitment or if they can be used
for a short period of time to reach a goal and then discontinue use.
Understanding GLP-1 Agonists
Before going into whether GLP-1 agonist medications are
needed for life, it's important to understand how these medications work. GLP-1
(glucagon-like peptide-1) is a hormone your body naturally produces in response
to eating. It helps to regulate your appetite by sending signals to the brain
when you're full, slowing down gastric emptying (the rate at which food leaves
your stomach), and helping control blood sugar by prompting insulin release
into the bloodstream.
Medications like Wegovy, Ozempic, and Mounjaro are synthetic
versions of this hormone. By mimicking
the effects of GLP-1, these medications help individuals lose weight by
reducing hunger and food intake, while also improving blood sugar control. This
makes them particularly useful for people with type 2 diabetes or for those who
struggle with significant weight issues that have been resistant to other
interventions like diet and exercise alone. Some critics suggest that they should
be a last resort, however proponents for their use point out the significant health
risks that are associated with obesity and that their use is both warranted and
beneficial in achieving improved health for patients.
The Weight Loss Potential of GLP-1 Agonists
One of the most appealing and interesting aspects of GLP-1
agonists like Wegovy is their ability to help people lose a substantial amount
of weight. Even patients with a long history of having tried and failed at other weight loss methods areIn the
STEP 1 clinical trial, individuals treated with semaglutide alongside lifestyle
interventions lost an average of 14.9% of their body weight over 68 weeks,
compared to just 2.4% for those taking a placebo. This trial included nearly
2,000 participants across 16 countries and showed that semaglutide was highly
effective in promoting weight loss in those with a BMI of 30 or higher (Wikipedia)
(AJMC). This level of weight loss is comparable to some
forms of bariatric surgery, making these medications a game-changer for those
struggling with obesity who now have the ability lose tremendous amounts of
weight without having to go through a risky surgery with a potentially painful
recovery.
For many people, the benefits of weight loss go beyond just
simple aesthetics. It’s great to look good which will boost confidence and make
someone feel better about themselves, however the health implications from
having a healthy BMI cannot be stated enough. Obesity is associated with a wide
number of serious health problems, including heart disease, diabetes, and
certain types of cancer. Losing a significant amount of weight can reduce the
risk of these conditions, improve overall health, and enhance quality of life. Not
only will you live longer, but you’ll live longer better. Better mobility,
energy, stamina and less health complications that overweight individuals
experience.
What Happens When You Stop Taking Wegovy?
While GLP-1 agonists are highly effective in helping many patients
achieve weight loss and reach their goals, an important question is “what
happens when you stop taking them?” One concern that many patients have while deciding
on whether or not to start a medically supervised weight loss program is
whether or not discontinuing the medication may lead to weight regain. This is
an important factor for many people deciding whether to start or continue these
medications long-term and one of the most common questions that we receive from
patients. One of the major flaws with hCG for weight loss is that besides not
being FDA approved, most patients not only regained the weight they lost on the
hCG diet, but even regained MORE weight than when they started. Also, it’s
thought that the weight gain from hCG is due to highly restrictive diets with
low daily caloric intake of only 500 calories. This is NOT the case with GLP-1
agonists.
Research has shown that weight regain is a real possibility
once any weight loss medications are discontinued, especially if zero lifestyle
modifications have been made. If one continues to eat and drink and have a sedentary
lifestyle, they will eventually regain some of the weight that they lost. In a
study published in the journal Diabetes,
Obesity, and Metabolism, individuals who stopped taking semaglutide
regained a t portion of the weight they had lost during treatment. On average,
participants regained two-thirds of the weight lost within a year of stopping
the medication when they made no attempt to eat better and exercise. However,
those who made some changes maintained a healthy weight while others were successful
on lower “maintenance” doses.
These outcomes highlight an important aspect of GLP-1
agonists: they do not "cure" obesity but rather manage it. The
underlying mechanisms that contribute to weight gain, such as genetic
predisposition, hormonal imbalances, and environmental factors, do not
disappear after stopping the medication. As a result, many people may find that
maintaining their weight loss is difficult without ongoing treatment and GLP-1
agonists are an excellent way to achieve these goals.
Is Wegovy a Lifetime Medication?
The idea of being on a medication like Wegovy for life can
be daunting, but it’s important to understand that the management of obesity,
much like other chronic conditions, often requires long-term intervention. There
is no quick fix to our health, especially something as complex as obesity. Obesity
is not just a matter of simple willpower; it is a complex medical condition
influenced by a variety of factors, including genetics, hormones, and behavior.
Our culture in the United States doesn’t help, as we are constantly surrounded
by fast foods and our supermarkets are filled with processed foods and
unhealthy options. For many people, lifelong management is necessary to
maintain a healthy weight and prevent the medical complications that are
commonly associated with obesity such as Type 2 Diabetes, Hypertension (High
Blood Pressure), Heart Disease, Sleep Apnea, Osteoarthritis, Fatty Liver, Stroke,
Certain Cancers, Gallbladder Disease and Chronic Kidney Disease to
name a few.
However, this doesn't mean that everyone who starts a GLP-1
agonist will need to be on it forever. The need for long-term medication
depends on several factors, including:
- Individual
Health Goals: Some people may achieve their desired weight loss and,
with the help of lifestyle changes, be able to maintain that weight
without the continued use of medication. We have found that most patients
are able to achieve their weight loss goals in a relatively short period
of time and maintain it. Granted a patient with a goal of 30lbs will have
an easier time than a patient who needs to lose 200lbs, but this is a
general trend that we have seen with our patients.
- Underlying
Health Conditions: People with obesity-related health conditions such
as type
2 diabetes, hypertension,
or sleep apnea
may benefit from long-term use of GLP-1 agonists to manage both their
weight and these conditions. The importance to long term health may
require a more stringent protocol and approach and has more serious
consequences with additional comorbidities.
- Lifestyle
Changes: The ability to maintain weight loss without medication
largely depends on adopting sustainable lifestyle changes. These include
healthy eating, regular physical activity, and addressing emotional and
behavioral factors related to weight management. Something as simple as
portion reduction and reducing the number of alcoholic drinks a week can
have a big impact. Knowing the
right foods to eat to help lose weight is important. A healthcare
professional may be able to offer valuable insight into strategies that
can play a major role.
- Personal
Response to Medication: Some individuals may respond better to GLP-1
agonists and find that they help them maintain a healthy weight and manage
their health conditions long-term, while others may be able to discontinue
the medication after achieving their weight loss goals. Tirzepatide and Semaglutide
are similar with slight differences so which one a patient responds to is
largely in part due to genetic factors unique to the person.
Strategies for Discontinuing Wegovy
For those patients who wish to stop taking Wegovy or another
GLP-1 agonist like Mounjaro, it’s important to do so under the watchful care
and guidance of an experienced healthcare provider. Abruptly stopping the
medication without a plan may lead to weight regain, which can be frustrating
and disheartening especially after working so hard to lose it. Having a strategy
can prevent this and help maintain overall health and wellbeing.
If you’re thinking about discontinuing a GLP-1 agonist
medication, here are a few strategies to
help maintain your weight loss:
- Gradual
Discontinuation: Rather than stopping the medication cold turkey all
at once, your healthcare provider may suggest tapering off slowly. This
gradual approach gives your body time to adjust and helps minimize the
likelihood of weight regain.
- Focus
on Sustainable Lifestyle Changes: Long-term success in weight
management depends heavily on several different lifestyle factors.
Continue to focus on balanced, portion-controlled eating, regular
exercise, and behavioral changes such as managing stress and emotional
eating. This alone can help the majority of patients keep the weight off.
- Ongoing
Support: Maintaining weight loss is challenging, and ongoing support
can make a big difference. Consider working with a healthcare provider,
dietitian, or weight management specialist who can offer guidance and
accountability. They will most likely go over some of the strategies listed
above but also help keep you accountable.
- Regular
Monitoring: Keeping track of your weight, body measurements, and
overall health can help you stay on track and catch any weight gain early.
If you notice the weight on the scale creeping up, reintroducing the
medication may be an option or doubling efforts with diet and exercise can
keep you in the targeted goal zone.
Lifelong Medication: A New Perspective
For some patients, the thought of being on a weight loss medication
like Wegovy or Mounjaro for life may seem overwhelming. However, it’s important
to think of GLP-1 agonists in the same way we think of medications for other
chronic conditions, like diabetes or hypertension. The same way that individuals
with diabetes may need insulin for life or those with high blood pressure may need
long-term medication, those with obesity may need ongoing treatment to manage
their condition.
Obesity is a chronic, relapsing condition, and many people
find that medications like Wegovy are a valuable tool in their long-term health
strategy. By helping manage appetite, reduce caloric intake, and improve
metabolic function, GLP-1 agonists can be a key component in maintaining a
healthy weight and preventing the health risks associated with obesity.
Wegovy for Life: The Benefits and Drawbacks
Like any long-term treatment, using Wegovy or another GLP-1
agonist for life comes with both benefits and drawbacks. Knowing which
GLP-1 medication is best for you needs to be determined by speaking with an
experienced and knowledgeable medical provider.
Benefits:
- Sustained
Weight Loss: For those who struggle with maintaining weight loss
through diet and exercise alone, GLP-1 agonists provide a valuable tool to
help keep the weight off long-term which is unbelievably important.
- Improved
Health: By helping individuals manage their weight, GLP-1 agonists can
reduce the risk of serious health conditions like heart disease, type 2
diabetes, and certain cancers which are all life threatening. For those
with existing obesity-related conditions, these medications can also
improve blood sugar control, lower blood pressure, and enhance overall health
and well-being.
- Better
Quality of Life: Losing and maintaining weight can improve overall energy
levels, increase mobility, help with improving self-esteem, and overall
quality of life. Many people find that staying on a medication like Wegovy
helps them feel more in control of their health and weight.
Drawbacks:
- Cost:
GLP-1 agonists like Wegovy can be expensive, especially for those without
insurance coverage. The out-of-pocket cost for Wegovy can be very
expensive making long-term use financially challenging for some, although
the argument can be made that the costs associated with more serious
diseases and health complications from obesity
are more costly.
- Side
Effects: While GLP-1 agonists are generally well-tolerated, some
people may experience side effects such as nausea, vomiting, diarrhea, or
constipation. These side effects may be mild and decrease over time, but
for some individuals, they can be bothersome. Usually starting off on a low
dose and gradually increasing can be a good strategy to deal with these
side effects.
- Commitment:
Taking any medication for life requires long-term commitment and discipline.
Some patients may find it difficult to stay consistent with their
medication regimen or may not like the idea of relying on a medication for
their weight management. Self-control and dedication is key to improving your
health and maintaining weight loss.
Is Wegovy a Lifetime Medication?
Whether semaglutide, tirzepatide
or another GLP-1 agonist becomes a lifelong treatment option depends on each person’s
individual health needs, goals, and specific guidance from their healthcare
provider. For some people, GLP-1 agonists offer a long-term solution for
managing weight and preventing the complications and medical comorbidities that
comes with obesity. For other people, these medications may be a temporary tool
to help achieve weight loss
while making sustainable lifestyle changes.
It’s important to view these GLP-1 agonists as part of a larger,
more comprehensive weight management plan that should include healthy eating,
physical activity, and behavioral changes to maximize and sustain results.
While weight regain is possible after stopping these medications, a gradual
approach and ongoing support can help mitigate this risk and help achieve
lasting results.
Ultimately, the decision to stay on a GLP-1 agonist like
Wegovy or Mounjaro for life should be made in collaboration with your doctor,
considering your unique health circumstances and long-term goals. In the end,
being healthy is the most important factor to consider and the ultimate end
goal.
